The training holds. The food is reasonable. The annual physical comes back normal. And still the body is not cooperating the way it used to — and you are not imagining it.
Send My Free White PaperAcross U.S. adults measured on the full cardiometabolic picture, only 6.8% met the criteria for optimal health (O’Hearn et al., JACC, 2022, NHANES n=55,081).
Being metabolically off while doing everything right is the statistical norm, not the exception. Metabolic syndrome prevalence rose to 41.8% in 2017–2018 (Liang et al., Postgraduate Medical Journal, 2023, NHANES 2011–18, n=8,183).
Insulin sensitivity begins its steep decline about five years before diagnosis while fasting glucose only rises sharply from about three.
The gap between those two trajectories is the window a standard glucose test reads clean (Tabák et al., Lancet, 2009, Whitehall II, n=6,538).
There is no reliable read of metabolic health in the scale or the BMI built on it. Up to about 30% of adults classed obese by BMI are metabolically healthy.
A substantial share of normal-weight adults are not. Two people at the same weight can sit on opposite sides of metabolic risk (Engin, Adv Exp Med Biol, 2017). The scale is a poor proxy for what is actually happening underneath it, and none of the drivers behind the change appear on a standard panel.
Written for the person between 40 and 65 who is doing everything right and watching the same effort return a different result.
Why this is measurable years before a diagnosis — and why a clean fasting glucose does not rule it out.
The measurable drivers behind the change — fasting insulin and HOMA-IR, glucose regulation in context, visceral adiposity, lipid particle detail, and muscle status.
What a physician-led read measures that the annual physical skips — read against optimal ranges, not disease cut-offs, and never a self-assigned diagnosis.
Every claim in this paper ships with its limit attached. Here are the four limits.
A driver panel is not a diabetes diagnosis.
A driver panel characterises metabolic function; it does not diagnose disease. Insulin resistance is not a diagnosis and HOMA-IR is a risk-prediction threshold. Reading either one as a diabetes diagnosis would be factually wrong.
A single number does not carry the picture.
No one marker carries the picture, and the scale and BMI misread it in both directions (Engin, 2017). A single figure read alone tells you nothing you did not already know. The useful measurement is the driver panel.
Moving a marker has not been shown to change an outcome.
Structured resistance and impact training improved physical function and body composition in older adults with obesity (Mesinovic et al., 2025, n=49). That is what was measured — function and body composition, not a hard outcome — and we will say which one applies to you.
Measurement, not a product.
Nothing here is a product. What is on offer is measurement read by a physician against your own history. Pricing is discussed at the consultation, not on this page. We test first. Therapy only when indicated.
